WA_Foundations_HospiceAndHomeCareSnohomish County_Home_HeroMoney Follows the Person (MFP) is a program of the DHS Division of Aging and Adult Services.  This program assists individuals who are current Medicaid recipients, and who live in a qualified institution, to move back into the community and receive home and community-based services.

To qualify for the MFP Program a person must be in a qualified institution, such as a nursing home, for at least six months and be a current Medicaid recipient for at least one month.  This program is available for the elderly, adults with physical disabilities, persons with developmental disabilities, and adults with mental illness.

One of our MFP Case Managers will work with the individual to decide where they want to live and help develop a plan of transition to the setting of their choice.  If there is not an existing home or family arrangements are unavailable, we will work with the individual on available housing options in the community from which they may choose to live.

ACMC prides itself on working tirelessly for our MFP clients to ensure a smooth transition for them.  We do this, in part, by working with housing authorities, setting up installation of cable and telephone service, ensuring lights and water are turned on and furnishing the client’s residence with furniture, food, clothing and linens.
